New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
NHS Choices webpage gives information on how to register for patients without a fixed abode.
Waddesdon Surgery serves the whole of Waddesdon as well as its surrounding villages;
Quainton, Grendon Underwood, Springhill, Edgcott, Woodham, Kingswood, Westcott, Ashendon and Upper Winchendon.
Medical treatment is available from the date of registration. Please contact reception for further information.
Named Accountable GP for all patients
All practices are required under the terms of the GP contract to allocate all patients (including children) a named accountable GP who is responsible for patients overall care at the Practice and who will take lead responsibility for the coordination of all services required under the contract.
Having a named GP does not prevent you seeing any other doctor in the practice of your choosing. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
