If you wish to have copies of your medical records this will need to be requested in writing, this can be done by completing the form below
Access to Medical Records Request Form
If you would like to allocate another person to be able to access your medical records for you, you will need to complete a consent form. Once completed this will be saved and your medical records updated allowing us to provide the information you have detailed to that person. This is handy for obtaining test results, finding out appointment times and requesting prescriptions for patients who require additional support.
