Named Accountable GP

All patients are allocated a named, accountable GP who is responsible for their overall care at the practice ensuring that all work is carried out on their behalf. Please note that you are still entitled to speak to, or make appointments with a doctor of your choice.

The accountable GP for all new patients is currently Dr Duleepa Koralage

If you have a preference to a GP, a form is available for you to complete requesting a transfer. We will do our best to accommodate your request although this cannot be guaranteed due to the different workload of our doctors. If you would prefer not to have an assigned GP at all please tick the relevant box on the form. The form can be found HERE
