Your views are important to us

We would like to get patient feedback on the services we offer, we would like to encourage you all to take part in a quick patient survey.

Survey outcomes will be discussed with both the GP’s and also the Patient Participation Group to identify an action plan highlighting areas for development, review or change.

The questionnaires are confidential and we will not be able to identify who has completed them.
