Prescriptions

Please allow 4 full working days (5 at busy times of the year) for prescriptions to be processed and remember to take weekends and bank holidays into account.

Repeat prescriptions can be requested in several ways:

• You can use one of the online platforms such as Patient Access, Evergreen Life or the NHS App 
• You can complete the prescription request form below
• You can drop a request in at the surgery

Please ensure you state your name, date of birth and specific names of the medications you require if using the below form.
